We read with great interest the recent article by Yu et al. (2025) [1] in the European Journal of Internal Medicine, which investigated the association between central systolic blood pressure (cSBP), cerebral small vessel disease (CSVD), and cognitive dysfunction. The authors’ efforts to highlight the value of central hemodynamics in understanding cerebrovascular pathology are commendable and represent a valuable contribution to vascular aging research. That said, we wish to respectfully raise key methodological aspects worth considering that could further refine the interpretation and generalizability of the findings.
