We read with interest the recent article of Bouzas-Mosquera et al., who investigated the impact effect of cardiac troponin I (cTnI) values on the frequency of myocardial ischemia and subsequent coronary events in patients with acute chest pain and biomarker concentration within the normal range [1]. The evidence emerged from this well structured study underpins a number of critical issues.
