Heart failure is thus responsible for more than 100,000 hospitalizations per year in France [1]. It accounts for 5% of all hospitalizations and is the main cause of hospitalization among elderly subjects. Some of these hospitalizations could be avoided if patients took greater responsibility for their disease and were followed up better [1]. This last point has been particularly well documented in heart failure and diabetes. Telemedicine may be of aid in this setting. Indeed it may even optimize the management of such chronic diseases, particularly by preventing emergency and repeat hospitalizations [2].
