Frailty is defined as a decrease of physiologic reserve and ability to maintain homeostasis leading to an increased vulnerability to adverse events [1]. Frailty is prevalent among elderly subjects and international guidelines on the management of valvular heart diseases strongly recommend an accurate assessment of frailty in the decision-making process for patients with symptomatic severe aortic valve stenosis (AS) [2]. Therefore, frailty needs to be taken into account by the Heart Team as it can drive the choice between surgery, transcatheter aortic valve implantation (TAVI) or conservative management.
