We appreciated the editorial by Garattini et al. [1], about the redesign of the Emergency and Urgency system in Europe. In relation to this argument, we would like to underline some critical aspects of the Emergency Care in the Lombardy Hospitals. Three years after the coronavirus disease 2019 (COVID-19) pandemic, which put the internal medicine departments to the test with great work and excellent results, the point is that without Internal Medicine the pandemic would have been a massacre [2]. Nonetheless today we are suffering from a significant increase in boarding hospitalizations in internal medicine [3].
