Chronic lung allograft dysfunction (CLAD) and pulmonary chronic graft-versus-host disease (Pc-GVHD) represent the major late non-infectious pulmonary complications following lung and allogeneic hematopoietic stem cell transplantation (HSCT), respectively. In both entities, diagnosis and phenotypic classification continue to rely predominantly on pulmonary function testing (PFT) [1,2]. Sara Piciucchi et al. have made an innovative point by applying the 2019 International Society for Heart and Lung Transplantation (ISHLT) CLAD diagnostic criteria to Pc-GVHD [1–3].
