The Current healthcare system in the tertiary hospital has become highly specialized to focus on patients’ organ-specific problems, and patients expect to receive such specialized care [1]. On the other hand, this “siloed” healthcare system has several negative aspects including interspecialty conflict and insufficient management of patients with multiple comorbidities, which can lead to medical error [1,2]. General internal medicine (GIM) departments solve these problems through a holistic and patient-centered approach [1], and they are also expected to perform as diagnostic specialists under the emerging concept of consultative medicine to help patients with perplexing conditions wandering in their diagnostic journey [3].
