An estimated 32 % of the European population live with multiple long-term conditions (MLTC), defined as two or more chronic conditions [1]. Patients with MLTC require integrated, tailored care, which extends beyond their individual diseases [2,3]. Tailored care places the patient at the center, and considers their physical- and cognitive functioning, psychosocial context, as well as personal goals, priorities and preferences [2,3]. However, current delivery of healthcare is often disease-oriented and fragmented, which can have negative outcomes for patients with MLTC.
