A 69-year-old Japanese man, who lived alone and was undergoing treatment for diabetes at another hospital was found unconscious in an unheated room at his residence in February 2025. He was transported by ambulance to our hospital. On arrival, his core temperature was 26.1 °C; systemic blood pressure, 156/124 mmHg; heart rate, 43 bpm; and oxygen saturation, 91 % (on 10 L/min oxygen via mask). Blood test results revealed hyperglycaemia (glucose, 374 mg/dL; glycated haemoglobin, 8.8 %), kidney failure (creatinine, 3.86 mg/dL), liver injury (alanine aminotransferase, 114 U/L), and inflammatory reaction (C-reactive protein, 32.80 mg/dL) resulting from pneumonia, as detected on a chest radiograph.
