A 73-year-old man presented to our dermatology department with a four-week history of palpable purpura on the lower limbs. His medical history included a suspicion of polymyalgia rheumatica based on a five-month history of inflammatory back pain, weight loss, fatigue, and night sweats, for which he was receiving oral prednisolone (20 mg/day).
