We read with great interest the article by Mannheimer et al. investigating mortality and causes of death in patients hospitalized with hyponatremia using a large propensity-matched cohort from the Stockholm Sodium Cohort [1]. The authors offer an impressive analysis grounded in robust registry data and modern causal inference tools. However, several critical methodological and interpretative limitations merit further discussion, particularly those which may compromise the validity of their conclusions.
