An 81-year old man was admitted to an Internal Medicine Division because of severe watery diarrhea lasting 8months, unresponsive to fasting and to a gluten-free diet. Since the onset of symptoms he had lost 8kg. He denied nausea/vomiting, hematochezia/melena, abdominal pain, hyporexia, fever, itching or night sweats. His past medical history was unremarkable; he never traveled outside Europe.
