A 61-year-old woman reported a right ocular painful redness with photophobia, two weeks after a rheumatoid arthritis flare. She was daily treated with leflunomide. Visual acuity was at 20/250 on the right eye. Ocular examination found a crescentic corneal ulcer of 6 mm (delimited by the two white full arrows, Panel A) in which a corneal perforation in the center revealed the underlying iris (white dotted arrow), in association with dilated perilimbal vessels (black dotted arrow) and a limbal edema (black full arrow).
