We read with great interest the study by Vauterin et al. investigating the association between guideline-directed medical therapy (GDMT) and mortality in patients with heart failure (HF) hospitalized for exacerbation of chronic obstructive pulmonary disease (ECOPD) [1]. Including >14,000 patients, the authors found only 12% of patients were treated with GDMT for both HF (defined by combined treatment with betablockers and renin angiotensin aldosterone system (RAAS) inhibitors) and COPD, whereas the risk of all-cause mortality was lower in patients treated with GDMT both during index hospitalization and long-term follow-up.
