Hospital readmissions among patients with multiple chronic conditions (MCC) represent a persistent challenge in European healthcare systems, with rates approaching 20 % in Spain and similar figures across Europe [1]. While considerable research effort has focused on developing highly accurate prediction models, our recent findings from the Re-Pluris project suggest it may be time to reconsider what constitutes clinical utility in readmission risk stratification for this vulnerable population [2].
