We read with great interest the article by Vitacca and colleagues regarding the persistence of respiratory events and daytime sleepiness in patients treated for obstructive sleep apnea [1]. The study addresses the pervasive clinical dilemma of the patient who remains symptomatic despite apparent therapeutic compliance and the investigation provides a valuable longitudinal perspective on treatment adaptation. While the reported findings regarding the prevalence of residual events are encouraging, several critical issues warrant further discussion.
