Calsolaro V and colleagues indicated significant benefits of oral anticoagulants (OAC) in oldest-old patients with atrial fibrillation (AF) in the previously published article, entitled “Long-term effectiveness and safety of anticoagulation therapy in oldest old, frail people with atrial fibrillation [1].” Since AF is the most common sustained cardiac arrhythmia related with worse clinical outcomes [2] and is increasing in prevalence and incidence worldwide in this ageing society [3], clarifying the best care for these patients is crucial.
