Sebastian-Valles et al. reported that a prehospital-to-hospital lactate ratio <1.23 identifies a subgroup of patients with severe hyperglycemic emergencies at markedly higher risk of in-hospital death (57.3 % vs 14.5 %). In a cohort of 128 patients (median age 71 years; mortality 34.4 %), the association remained strong after adjustment, and the authors suggest that this readily available metric could support early risk stratification in EMS practice [1].
