Recently, the American College of Cardiology (ACC) and American Heart Association (AHA) with scientific joint committees published the new clinical practice guidelines of acute pulmonary embolism (PE) [1,2] The new 2026 ACC/AHA guidelines for acute PE have introduced the new ACC/AHA PE clinical categories (Category: A-E), which could enhance the precision of rapid severity classification of acute PE. Unlike the European Society of Cardiology (ESC) severity classification of acute PE [3], the new ACC/AHA classification primarily relies on clinical presentation and hemodynamic status, aiming to facilitate immediate risk assessment.
