We read with great interest the study by Aboubakr et al [1] investigating the impact of brain biopsy timing on diagnostic yield and clinical outcomes in patients with neurological diseases of unknown etiology. Including 294 patients over a 16-year period, the authors demonstrated that early biopsy (≤ 1 month) independently predicts better 6-month functional status (OR 2.03) and higher diagnostic success [2]. This large single-center cohort provides important real-world insights into a clinically challenging population where tissue sampling is frequently delayed.
