We were pleased to read the commentary by Shen et al. [1] dwelling upon our work that investigated the prevalence of orthostatic hypotension (OH) in patients with heart failure (HF) [2]. Their letter raises several thoughtful and clinically relevant considerations regarding OH in HF. We particularly appreciate their emphasis on the dynamic nature of OH and the importance of considering the clinical context in which orthostatic blood pressure is assessed. We fully agree that moving from a simple prevalence estimate toward a more refined characterization of OH phenotypes represents an important direction for future research.
