We read with interest the pooled analysis by Marques and colleagues examining polycythemia and cardiovascular outcomes across five randomized trials of sodium-glucose cotransporter 2 inhibitors (SGLT2i) [1]. Among 26,064 participants, polycythemia was present in 2.8% at baseline and 6.3% at week 52, and randomization to an SGLT2i was associated with more than sixfold higher odds of polycythemia at week 52. The authors further reported no statistically significant treatment-by-polycythemia interaction and concluded that SGLT2i benefit appeared preserved in patients with pre-existing or treatment-emergent polycythemia [1].
