To the Editor Coscetta and colleagues provide the first systematic estimate of orthostatic hypotension (OH) in patients with heart failure (HF), pooling nine studies (10 cohorts; 2036 patients) and reporting a prevalence of 26.7% (95% confidence interval 20.4–34.2%) [1]. This is clinically important because orthostatic blood pressure is easy to measure and concern about hypotension often influences implementation of guideline-directed medical therapy (GDMT). The authors appropriately acknowledge substantial heterogeneity (I²=90.2%) and the limited evidence linking OH to treatment delivery and outcomes [1].
