A 40-year-old woman presented with left lower abdominal pain that had begun 2 days earlier and subsequently shifted to the left hypochondrium. The pain was persistent, worsened with deep inspiration and when lying on her right side, and improved when lying on her left side. She was on day 5 of her menstrual cycle. Her husband was her only sexual partner. She had never been pregnant or given birth and reported no dyspareunia. Her vital signs were normal. Physical examination revealed tenderness in the left hypochondrium.
