Heart failure with preserved ejection fraction (HFpEF) represents a growing and increasingly prevalent phenotype of heart failure (HF) in older adults, yet its recognition in routine clinical practice remains challenging. In fact, in older patients the distinction between physiological cardiac aging and pathological dysfunction is often blurred, and symptoms and signs lack specificity in the context of multimorbidity [1].
