We read with great interest the article by Ishihara et al. evaluating the survival impact of early high-flow nasal oxygen (HFNO) following extubation in intensive care patients [1] Although the authors conclude that their newly introduced extubation criteria reduced mortality and reintubation rates, several methodological, statistical, and ethical issues substantially undermine the reliability of these findings.
