A 56-year-old woman with allergic rhinitis presented to the emergency department with severe lower abdominal pain and generalized wheals shortly after lunch. Blood tests and abdominal CT were unremarkable. She was diagnosed with anaphylaxis, treated with intramuscular epinephrine, and admitted for observation. On examination, resolving hives and painless bilateral mandibular swelling were noted.
