A 51-year-old white man presented to the internal medicine department with altered general condition and significant weight loss of 14 kg in 3 months. The main symptom was cervical pain and headache with inflammatory rhythmic pain. Fever was not reported. Urine dipstick showed isolated hematuria. Biological findings revealed an inflammatory syndrome with a C-reactive protein (CRP) level of 104 mg/L, hypergammaglobulinemia, thrombocytosis and augmented ferritin. Repeated blood cultures were negative.
