Vedovati and colleagues provide a timely analysis of central embolic localization in acute pulmonary embolism (PE) using the large, multicentre Contemporary Management of Acute Pulmonary Embolism (COPE) registry [1]. Their observation that central emboli were associated with PE-related death, but not with all-cause mortality, is clinically interesting. The question is whether this association adds enough information to change risk classification or treatment decisions.
