The multicentre, prospective study by Russo et al. [1] evaluated a deprescribing strategy supported by ambulatory blood pressure monitoring in 92 hypertensive patients with severe hypotensive syncope. The mean number of hypotensive medications decreased from 2.4 to 1.3 per patient, while mean 24-hour systolic blood pressure (SBP) increased from 118.8 to 127.5 mmHg. Daytime SBP drops below 90 and 100 mmHg decreased by 52% and 53%, respectively, and the authors reported a 66% reduction in syncope burden.
