The new guidelines on the management of peripheral arterial and aortic diseases have been released at the European Society of Cardiology (ESC) Annual Congress 2024 and published simultaneously [1]. The previous version dated back to 2017 [2]. Today more than then we must recognize the authors for a truly remarkable work and congratulate them for it. The aim is to represent in a single document the complexity of the approach to a system such as the arterial vascular system, characterized by interconnections and profound reciprocal influences at the cardiac level, the supra-aortic vessels, the renal circulation, the aorta and the peripheral arteries.
