A 22-year-old man was referred to the dermatology service for the appearance of thickening of the skin, predominantly on the hands, feet and face, accentuation of the folds of the forehead and deformation of the hands (Fig. 1). These symptoms appeared at the age of 18 and progressively worsened. His medical history included inflammatory peripheral polyarthralgia.
