A 32-year-old Hispanic man presented with a one-month history of bilateral infrapatellar tender erythematous subcutaneous nodules (Fig. 1, a and b), accompanied by dysesthesias in the same area and palms, and intermittent lancinating pain. His medical history was unremarkable, although he reported a similar episode eight years prior, when a doppler ultrasound showed reduced blood flow in the left saphenous vein, leading to saphenectomy at another institution; lesions resolved after the procedure, but he experienced recurrent episodes with spontaneous remission, which had previously not prompted medical attention.
